St. Michael Catholic School is an Approved Indiana Choice Scholarship School
Parents interested in applying for this scholarship can visit the Indiana Department of Education website. This is the only site that should be used to collect information for the Scholarship Program. Families can obtain information on income eligibility.
Applications are made directly through our Scholarship & Grant Coordinator, Al Balcer. IDOE required papers must be submitted at the time of school application. The following information must be provided at application.
The IRS Tax return pages 1 & 2 for families to better understand what is needed.
Two things are especially critical with this form.
- Line 11, Page 1 – The amount in this line indicates the adjusted gross income of the family applying for the Scholarship. The amount in this line determines the eligibility of the family based on the 2024-25 Choice Income Guidelines in the new document you created.
- Dependents, Page 1 – The students applying for the scholarship are required to be listed as dependents in this section on the tax return. If the student is not listed in this section, the family must notify me and indicate the reason why. A special Indiana Choice form will need to be sent to and completed by the family to indicate why the student Is not listed on the tax return.

This scholarship is for current Indiana residents.
All students applying for admission to St. Michael Catholic School are required to take a placement test.
